Information for Seton Employee Benefits or Seton SmartHealth Employee Plan

Seton employees have one of two plan options:

• The first option is the PPO 500/15 plan. This plan allows you to receive therapy sessions for $15 (copay) each.

• The second option is the HDHP 1300/0 plan. This plan has a $1300 individual deductible or $2600 family deductible. After your deductible is met there is no charge for services for the remainder of the year.

Seton covers individual, couples, family, and group psychotherapy.
